White emergency contact card with blue ink

I lost my white emergency contact card yesterday around 3pm near the Starbucks on West Main Street in Rapid City. It's a small plastic card the size of a credit card, white with a thin blue border and blue print. On the front it lists my name, Priya Patel, and two emergency numbers including my mom's; on the back there's a tiny red heart sticker I added for luck. The card is a bit bent at the top-right corner from a fall, and it has sentimental value because it helps my family stay in touch when I'm traveling.
